2. Inpatient visit for cardiac surgery PDF Print E-mail

After you have a scheduled date for cardiac surgery, you may need to be admitted to the cardiothoracic surgical ward located on 4th FL , Sorkor Building for patient preparation.

Step-by-Step to admission process:
i.    Once you get the confirmed bed available, you should prepare the required documents as follows:

1.National ID card (Both original and copy needed)
2.Health Insurance Card/ National Health Care Card (Both original and copy needed)
3.MD Referral Document (Case of Civil Servant without automatic reimbursed billing)
4.King Chulalongkorn Memorial Hospital ID Card

ii.    Bring all documentation in #1 along with your medical record with  you or your family to the hospital
iii.   Go to Sor-Kor Building (map # 49)
iv.   Take the elevator to FL 4
v.    Once you get out of the elevator, the cardiac surgery ward is located on your right hand side. Please direct to the nurse station

 

What to bring:
Soap
Powder
Toothpaste and Toothbrush
Comb
Tissue Paper
Shampoo
Pampers (Especially for elderly, handicap, or dependent patients)

Bathrooms: Shared bathrooms
