Brugada syndrome (BS) is an arrrhythmogenic disease characterized by a structurally normal heart and an increased risk of sudden cardiac death from ventricular fibrillation. BS is found worldwide but it is the most common cause of sudden cardiac death in young men in Thailand and other countries in Southeast Asia. The disease has been prevalent in Northeastern Thailand. Most victims were healthy young men who died unexpectedly at night. The automatic implantable cardioverter defibrillator (AICD) is currently the only proven treatment that can prevent sudden death. In general, the incidence of VF in BS is low with the incidence of 2%-7% per year. However some BS patients may experience multiple recurrent episodes of VF and VF storm (3 or more episodes of VF per day). VF storm may result in death or give rise to serious clinical complications. Cardiac Center at King Chulalongkorn Memorial Hospital is the first to do the clinical study to treat the VF storm in BS 1. The abnormal area inside the heart that is responsible for the generation of VF storm was searched and mapped by using non-contact mapping system. Once the area was found the radiofrequency catheter ablation was performed. The treatment was safe and successfully suppressed the VF storm in long-term follow up.
